Subject: **Very Important Information--Guarantor Accounts**

Please note that due to budget constraints, as of 6/1/2008 the Clinical Resource Center (CRC) will no longer fund any inpatient or outpatient ancillary charges. These charges can no longer be charged to the R09 account. These charges are now the responsibility of the investigator. Each IRB approved protocol must have at least one guarantor account created for payment of study charges. It is the responsibility of the study coordinator to insure that the account(s) are set up.

At the very bottom of the web page above is a link to the document that indicates how guarantor accounts are created.

Subject: Important Information Regarding Your Amendments

ALL AMENDMENTS MUST BE SUBMITTED TO THE CRC ADMINISTRATIVE OFFICES (W556)
Please submit the following items:
1. Memo detailing the changes addressed to CRC/Dr. Weiss (this memo must be signed by the PI)
2. Copy of IRB Approval letter
3. Copy of all documents submitted to the IRB including: IRB amendment application and all revised documents i.e. protocols, consents,DSMP,etc (all changes to these documents should be highlighted). If no changes were made please state as such.
4. Updated budget reflecting changes from prior CRC-approved budget. If no changes were made to the budget, please state as such.



Subject: Important Information on Submitting Budgets

Please be sure to fill your budgets out completely. A complete budget will include the following information: (1) Name of tests, (2)Charge Code for requested tests, (3)Lab where tests are performed,(4,5,6) place an X in the appropriate box indicating who will be billed for tests,(7) # of subjects per YEAR, (8) # of tests per SUBJECT, (9)total number of tests to be performed per YEAR. If you have any questions in reference to completing your budgets, please contact Sonja Johnson-Hall 2-7008.

Subject: Please Revise Your Consent Forms!

The following change has been requested by the Research Subject Advocate, please include the following change with your next IRB amendment or if you are submitting a new study, please include during the initial submission process.

On your consent forms in the section titled, "Who Do I Call If I Have Questions or Problems?" , please add the following text: "Dr. F. Gary Toback and Connie Robinson, R.N., the Research Subject Advocates for the Clinical Resource Center (CRC), are available to talk to you if you have any questions or concerns. You can contact them by calling (773) 702-7008. If you are at the CRC in the University of Chicago, you can talk with a Research Subject Advocate in Room W-560."

Subject: Reporting Adverse Events

Subject: Submission Deadlines and Requirements

Deadlines:

Submission deadline: December 22nd for the January 21st meeting of the Internal Scientific Advisory Panel (ISAP)

Please send an email to sjohnso2@bsd.uchicago.edu to request that your new protocol be added to the website. Include the following information in your email request:
1. Study Title
2. PI, co-investigator(s) and study personnel names
3. IRB number (if already assigned)
4. Category (A=Research only; B=Research with Patient Care; D=Industry Sponsored)

Please make sure that you click on the "Make Submission" link after all the appropriate documents have been uploaded. You will then need to click on the "Submit" button on the following page. The system will let you know if your submission is successful.

Subject: Welcome to the CRC Submissions site!

This is the public news area for the CRC submissions site. Feel free to post comments, news items, etc here. You must login to post - if you do not have yet have your login, you can click on the 'Request login' link above to get one.

In order to complete a protocol submission to the CRC, the PI and all co-investigators listed on your protocols must submit a NIH biosketch and Other Support document in their "My Account" area, in addition to all the CRC documents required. If you are PI for a project, please confirm that the list of your co-investigators for each project is complete and up-to-date. In the next couple weeks, the co-investigators will be receiving an email alerting them to this site.

If you have anyone that you want to authorize to help manage your
protocol submission, eg a project manager, please have them complete a "Request Login" for at http://crcdocs.uchicago.edu/request_login . In the comment area of that web form, they should indicate which protocol they would like to be added to and why. Sonja Johnson-Hall will process these as they come in.
